Patient Information

Thank you for trusting our surgeons with your care. As a new patient, we need some information for your medical records. Please click on the Patient Intake Forms tab, print and complete all the forms and bring them to our office for your first visit. Or, you may print the forms and complete them in our office. We ask that you arrive 30 minutes early if you have not yet completed the new patient forms.

Please make appointments should as far in advance as possible. We will accommodate you in the event of an emergency. If you are unable to keep an appointment, please give us 24-hour notice to allow us to schedule other patients.

Although we do our best to remain on schedule, occasionally, unforeseen circumstances may occur that cause us to delay or postpone your appointment. We will notify you in advance of any such delays. While in our office, we strive to minimize your wait time. We have several surgeons who work simultaneously in our office, so you may see others called back to their appointments before you even if you’ve arrived first. They are likely seeing a different doctor. If we fall behind schedule due to an emergency, we will inform you and allow you to continue waiting or give you an option to reschedule.

We are participating providers for Medicare and many managed care networks and will accept your co-insurance or co-payment at the time of your appointment and will bill your insurance directly. Please bring your current insurance card(s) with you so that we can assist you in determining your benefits.
